Release and waiver of liability.

In consideration of my child’s participation in the playroom, bookstore, and party activities, I, the undersigned, on behalf of myself, my

heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue The Bluebird Social, its officers, directors, employees, volunteers, agents, and representatives (collectively, the "Released

Parties") from any and all liability, claims, demands, actions, or causes of action, of any kind or nature, whether

known or unknown, arising out of or related to any loss, damage, or injury, including death, that may be sustained by

my child, or to any property belonging to my child, while participating in activities or while on the

premises where the activities are conducted.

INDEMNIFICATION:

I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, actions, suits,

damages, liabilities, and costs, including reasonable attorney's fees, arising from my child’s participation in

activities.
